Episode Synopsis

The group is wondering where Ethan has disappeared to. Jalen and Lauren had not seen him for a few days, but since all three of them worked the two just assumed that Ethan had been working nights at the clinic and that was why he had not been around when they were at the apartment. Now they find that he has not been at work for a couple of days.

The scene switches to a hospital, several years ago. Ethan is working as an intern and has just gotten off shift. His fiance, Julie, shows up to pick him up for their dinner date. As the two walk down a hospital corridor the scene shifts to a different hallway in a very different hospital. This hospital is dark and decayed. In a poorly lit room we see Ethan strapped down to an operating table. Hovering over him is the apparition of Walter Blake, who is attempting to "treat" Ethan with a variety of archaic and brutal instruments. Ethan's screams echo throughout the decaying building.

The group splits up to try and find any leads as to Ethan's whereabouts. While Patrick and Jalen conduct an investigation on the street, Dana and Lauren visit Anne Steele's youth shelter. They ask if there has been any sign of Ethan, but Anne has not seen him since he and Dana had helped her earlier. While at the shelter Lauren receives a vision of a man in Victorian clothing being subjected to brutal treatments in an insane asylum.

We now see that the decaying building is the asylum from Lauren's vision, though it has obviously been abandoned for quite a long time. Inside Walter Blake is continuing his "treatments" on Ethan. Blake's questions lead Ethan to remember when he first gained his second sight. The scene flashes back to Ethan and Julie driving to their dinner date. As they are driving an earthquake hits and their car is buried under a collapsed highway overpass. Ethan is hurt but conscious and keeps talking to Julie, who appears alright. When the rescuers finally arrive Ethan is horrified to learn that Julie had been killed instantly in the crash and that he had been speaking to her ghost the whole time.

Based on Lauren's vision, Dana is able to dig up information on the asylum. The Maylum Asylum was built in the late 1800s, where there was a patient named Walter Blake who matched the description of the man from Lauren's vision. The asylum was later renamed the Maylum Institute for Mental Health, but closed down in the 1960s. Dana and Lauren learn that the abandoned building still exists in the older part of Los Angeles and there have been several reports over the years of missing persons turning up near the old asylum, and that all of those found there had invariably been driven insane by something.

Meanwhile, Patrick and Jalen's investigation has led them to a bar in one of LA's demon ghettos. Their questions lead to an altercation in the bar with twin demon brothers. Patrick and Jalen are able to take out the brothers, and learn that a man in a suit hired the demons to kidnap Ethan and drop him off in the old part of LA. When they compare notes with Lauren and Dana they realize that the demons took Ethan to the abandoned asylum.

Back at the asylum, Walter's brutal torture of Ethan continues. The scene shifts to the past again, where we see Ethan being called into his supervisor's office. The supervisor tells Ethan that his work since Julie's death has been getting worse and that the hospital can no longer allow him to continue his internship there. By this point Ethan had already been stealing drugs from the hospital to help him cope with his loss, and his new sixth sense. Without proof, the hospital cannot charge him with anything, so instead they fire him and get him expelled from medical school.

The rest of the group arrives at the abandoned asylum and go in to search for Ethan. Immediately Walter senses their arrival and begins using his ghostly poltergeist powers to rearrange the hospital's interior to split up the group so he can begin "treating" them as well. Ethan's flashback continues and we see him as he starts his new job at the clinic. As they are underfunded, the clinic will take anyone they can get, even someone with Ethan's background. The scene ends as Ethan is given the key to the clinic's drug cabinet.

Meanwhile, Dana has been separated from the group and is confronted by Walter. Dana turns the tables on the apparition when she uses an improvised blood ritual to temporarily bind Walter's ghost to a single room in the asylum. This gives the others time to find Ethan and Lauren uses her telekinesis to smash the group an exit from the haunted asylum.

The scene shifts to Wolfram & Hart's offices where Travis McCauley is on the phone with Leland Richter. McCauley updates Richter on what Wolfram & Hart have been doing about the group's interference in Richter's affairs.
